The Comprehensive Osteopathic Medical Licensing Examination (COMLEX) is the osteopathic equivalent to the United States Medical Licensing Examination (USMLE), which allopathic students take. Osteopathic students are in a unique position when it comes time to take your board exams. While they must complete the COMLEX-USA Levels 1, 2, and 3 to receive licensure as a DO, osteopathic students also have the opportunity to take the USMLE as well.
Every student starts from a different point, so it’s impossible to guarantee an exact score increase. It consists of approximately 420 multiple-choice questions and 26 computer-based clinical decision-making (CDM) cases. Day 1 focuses on foundational and clinical science knowledge applied to patient care, while Day 2 emphasizes advanced clinical problem-solving, diagnosis, and management in diverse healthcare settings. Content spans all major disciplines—including internal medicine, surgery, pediatrics, OB/GYN, psychiatry, emergency medicine, public health, and osteopathic principles/OMM—with attention to continuity of care, patient safety, and health systems. Scoring is Pass/Fail, confirming the ability to provide safe and effective osteopathic medical care without supervision.
The COMLEX-USA Level 2-CE is usually taken during the fourth year of medical school and is a 9-hour exam consisting of 352 multiple-choice questions, divided into 8 sections of 44 questions each with scheduled breaks after every two sections. The exam emphasizes clinical decision-making and problem-solving, testing the application of medical knowledge in patient presentations and scenarios.
